Defisit keuangan BPJS Kesehatan terus membesar dan dikhawatirkan berdampak pada pelayanan kesehatan masyarakat. Tahun lalu defisit BPJS Kesehatan mencapai Rp 10,4 triliun. Tahun ini defisit membengkak hingga Rp 16,5 triliun.
Direktur Utama BPJS Kesehatan Fachmi Idris mengatakan masih ada triliunan rupiah utang BPJS Kesehatan kepada rumah sakit dan farmasi. Sementara, ada denda 1 persen per bulan yang harus ditanggung BPJS Kesehatan bila telat membayar tagihan kepada para mitranya.
Bahkan, Jokowi menegur Menteri Kesehatan Nila F Moeloek dan Dirut BPJS Kesehatan Fachmi Idris. 'Mestinya sudah rampunglah di (tingkat) Menkes, di Dirut BPJS. Urusan pembayaran utang rumah sakit sampai Presiden. Ini kebangetan sebetulnya,' kata Jokowi, 17 Oktober 2018.
Ada beberapa faktor yang membuat defisit, salah satunya iuran BPJS Kesehatan belum naik sejak 2016. Penerima Besaran Iuran (PBI) atau peserta BPJS yang disubsidi pemerintah hanya Rp 23 ribu per orang. Hasil perhitungan aktuaria dan rekomendasi dari Dewan Jaminan Sosial Nasional (DJSN) pada 2016, seharusnya iuran tersebut mencapai Rp 36 ribu per orang. Kemudian peserta yang Bukan Penerima Besaran Iuran (BPBI) kelas II Rp 51 ribu, dari yang seharusnya Rp 63 ribu.
